Most meetings open with a reading, someone shares for a while, and others speak if they want to. It lasts about an hour, and it is free, though a basket goes round for the room hire. Nobody takes a register and nobody will ask you to commit to anything. An open meeting can be attended by anyone, including family. A closed meeting is for people who think they may have a problem themselves. If the first group does not suit you, the next one along will feel different, because each has its own character. The usual advice is to try three before deciding it is not for you. Knowing the stages helps you judge whether a particular service, wherever it is based, is offering a genuine treatment plan rather than just a room and a routine, and it also helps you compare one type of rehab against another on the same terms. - Assessment. A clinical or psychological evaluation of substance use, any physical dependence, and any co-occurring mental health condition, sometimes called a dual diagnosis, so the treatment plan actually fits the person rather than a generic template. - Detox, where needed. Medically supervised withdrawal from alcohol, opiates, benzodiazepines or other drugs, managed to keep withdrawal symptoms safe and as comfortable as possible. Not everyone needs this stage; many people move straight to therapy. - Therapy. A residential or community programme typically combines individual therapy with group therapy or group psychotherapy, and draws on established behaviour therapy approaches such as cognitive behavioural therapy (CBT) and, in some services, dialectical behaviour therapy (DBT), alongside complementary work like art therapy or drama therapy that helps people process experience without always needing words. - Family and relationship work. Where appropriate, family therapy or family sessions bring partners, parents or children into the picture, since addiction rarely affects one person in isolation. - Aftercare and long-term recovery. A responsible programme does not end at discharge. Ongoing support, individual counselling, relapse-prevention planning and a route back into support groups such as AA or NA are what turn a short stay into long-term recovery rather than a pause, and what keeps the journey to recovery going once the formal programme ends. Underneath all of these stages, the aim is the same: to help individuals get back to a stable, functioning life, not simply to get someone through withdrawal and out of the door. None of this is about a single therapy or a single recovery model being “the best”. It is important to remember that rehab, of whatever kind, works by combining the right mix of these elements for the person in front of you, and a good rehab centre should be able to explain plainly which of them it actually offers, rather than treating an extended rehab programme as automatically better than a short, focused one. ### Therapy for alcohol and drug addiction, and how addiction can affect your life Addiction is increasingly understood, including by the National Health Service (NHS) and by psychiatry more broadly, as a condition that involves the brain’s reward system as much as willpower. Repeated substance use changes how the reward system responds to ordinary stimulus, which is why thought patterns, cognition, mood and behaviour can all be affected alongside physical health, and why recovery is rarely just a matter of deciding to stop.
