# Luxury rehab in Spain: how to find the right one

All you need to know about the luxury end of the Spanish market: what it actually costs, what the money is buying, which of the therapies on the list have evidence behind them, and how to check any centre on the public health register in two minutes.

Luxury rehab in Spain runs from about €4,500 a week for a villa programme to around €47,380 and as much as €100,000 a week at the ultra-private end, in Mallorca, Ibiza and around Marbella, which is €190,000 to €400,000 for a month. Above a certain point that money is not buying more treatment. Both are real things to want, and worth paying for if you want them, so it is worth knowing which of the two you are buying. It is buying privacy, a higher staff-to-client ratio and a better building. The clinical week, which is what actually treats addiction, is often no longer than at a centre costing a tenth as much, and is sometimes shorter, because a programme built for one client at a time has no therapeutic community in it. ## What the luxury end of the Spanish market actually costs

These are published ranges from public listings and from centres’ own sites, not quotations. Every
centre sets its own fees, and many at the top of the market quote privately rather than publish a figure.

A rented or owned villa, a small staff and a programme built around one or two clients. Typically includes accommodation, meals, one-to-one therapy and some holistic sessions. Group work depends on how many clients are in residence at the time, so it is worth asking.

Published on its own site as an individually designed programme in a discreet villa in the hills. The bespoke tier is the same setting with the programme built around the individual.

Camino Recovery, Málaga province

Published on its own site as fully inclusive: residential treatment for addiction, trauma and co-occurring conditions, using CBT, EMDR, equine therapy, mindfulness and family work. Detox is not on site and is arranged with a local provider beforehand.

Sierra Recovery, Humilladero, Málaga

£9,900 for 28 days

All rooms are individual, and the price is published up front. Three structured therapy groups a day, five days a week, individual therapy three times a week, and a seventeen-module psychoeducation curriculum. Equine and holistic work on the land. All meals cooked by a Michelin-trained chef. A therapeutic community, with a group of other adults in the house. A full year of aftercare, one session a week, more than 48 in the twelve months after you leave, with a written handover to your GP. The range is very wide, and it is worth knowing what moves along it. What changes as the figure rises is
the property, the privacy and the ratio of staff to clients. Those are real things, they are worth paying for
if you want them, and they are a different purchase from the clinical week. Knowing which one you are buying is
the useful part.

## What you are actually buying when the price goes up

- Privacy and discretion. Genuinely valuable if you are recognisable, or if your work makes
exposure a real risk. This is the most honest reason to pay a large premium.

- Staff-to-client ratio. More one-to-one hours. Worth something, up to a point, and past
that point more individual therapy is not better therapy.

- The building and the location. A sea view, a private pool, a chef. Pleasant. Not
treatment.

- Privacy from other clients. Valuable if discretion is the priority. It also means there
is no therapeutic community, which is a real clinical trade-off. See below.

## The therapy list, and which parts of it have evidence

The single most common way to justify a higher price is to make the list longer. Here is what is actually on
those lists, and what the evidence says about each in the treatment of addiction specifically.

Genuinely useful for regulation, routine and getting out of your own head. We offer all of them. They support the clinical work rather than replace it.

Massage, spa, personal training, gourmet dining

No evidence in addiction treatment

Pleasant, and sometimes restorative. They sit alongside treatment rather than count as it.

IV vitamin drips, ozone, cryotherapy, "biohacking" panels, hyperbaric chambers

These appear on some treatment menus. None of them has evidence as a treatment for addiction, and some carry risks of their own, so it is worth asking what a centre claims for them.

"Bespoke, one-to-one, fully individualised programme"

Depends entirely on what is in it

Sometimes it means genuinely tailored care, and sometimes it means the week is arranged as it goes. Either way, ask for the written week.

Nothing in the lower half of that table is a criticism. We offer equine work, holistic

rather than substitute for it, so it is worth knowing what share of the fee is buying the clinical week.

## What a therapeutic community adds

A one-client programme, by its nature, has no therapeutic community in it. That is worth saying plainly,
because it is the one part of residential treatment that money cannot assemble for a single person.

Most of what changes in residential treatment happens between the sessions. It happens in a group where
somebody else has already used the excuse you were about to use, and says so. It happens at a dinner table where
you are known, over four weeks, by people with no reason to be impressed by you. It happens in the ordinary
hours, practising a normal day without the substance, in front of witnesses. None of that can be bought
individually, and no amount of money reproduces it for one person alone in a villa with staff.

If discretion is the reason you are looking at a private villa, that is a real and legitimate reason. It is
worth knowing what it costs clinically, so you can decide with the trade-off in front of you rather than behind
it.

## How to check any Spanish centre is licensed, in two minutes

This is the one claim on any rehab website that you can settle for yourself, ours included, and it takes about two
minutes. In Andalucía a residence delivering addiction treatment needs its own N.I.C.A. authorisation for
care unit U.71, held by that centre at that address. A tourist-let permit, a company number or a visiting
clinician’s own registration covers something else.

In Spain a residential centre treating addiction is a health centre. It
must be authorised by the health department of its autonomous community and entered on a public register, and
that authorisation is renewed and inspected.

- Ask for the registration number and the register it sits in. In Andalucía that is
an N.I.C.A. number, in Madrid a CS- number, in the Comunitat Valenciana an ACD/ number for drug-dependency
services. In SICESS, search by tipo de centro and select U.71 Atención sanitaria a drogodependientes, or the search will not return the centre.

- Search it yourself. Regionally, or in
[REGCESS](https://regcess.mscbs.es/regcessWeb/inicioBuscarCentrosAction.do),
the national register kept by the Ministerio de Sanidad.

- Do not accept a substitute. A company registration number, a VAT number, a trade
association logo and a directory listing are all different things, and none of them is an authorisation to run
a health centre.

- Ask what the licence covers. Being registered is not the same as being licensed for
medically supervised detox. Most Spanish residential centres, including this one, are not detox units.

## Referral fees, and why to ask about them

Many people arrive at a centre through a referrer rather than by finding it themselves. Referral is an
established part of the sector, and the referrer is usually paid out of the fee the client pays, at rates
commonly reported at twenty to thirty per cent of the treatment cost. It is worth knowing that
this is how a lot of the market works.

On the figures in the table above, that share is a meaningful part of a fee. It is also worth knowing that a
referrer who is paid by some centres and not others is giving you advice with a commercial interest in it, which
is rarely presented as advertising.

The legal position differs by country. In the United States, paying or receiving anything of value for a
patient referral to an addiction treatment provider is a federal crime under the Eliminating Kickbacks
in Recovery Act of 2018, known as EKRA. In Europe there is no equivalent statute, and the practice is
legal in Spain and in the United Kingdom. Since it is legal and normal, it is simply something to ask about
rather than assume.

Put it to any centre, in writing: does anyone receive a payment for referring me, and how
much?

## Where Sierra Recovery sits, plainly: affordable luxury

We are not the most expensive centre in Spain and we are not trying to be. Sierra is a restored cortijo on
forty hectares of olive groves, forty five minutes from Málaga airport. There is a pool, a courtyard and
horses on the land. It is a working cortijo rather than a five star hotel, restored and deliberately left rustic, and that is what we show you. For a month of real work it turns out to be the better building.

What we do mean by luxury is the things that are actually scarce in treatment. There are
three in particular, and they are the ones we have kept in:

- Your own room. Having a door you can close in the first difficult week is a clinical
need, not a hotel extra. It is the difference between being able to fall apart privately and having to hold it
together in front of someone else. All rooms here are individual.

- One-to-one time. Individual therapy is the most expensive hour a centre delivers, so it
is worth asking any centre how much of it you get. Here it is three sessions a week, every
week.

- Food worth eating. Appetite is one of the first things to come back and one of the first
things people notice. Meals here are cooked by Harri Archanco, who trained at the Michelin-starred Galvin at
Windows in London. Every day, not as an event.

That is what affordable luxury should mean: not marble and a sea view, but the parts of the experience that
are genuinely rare, and keeping them in.

What is genuinely high specification is the clinical week: three structured therapy groups a day,
five days a week, individual therapy three times a week, a seventeen-module psychoeducation curriculum,
and the same clinicians treating you from the first day to the last. A private room is included as standard, not
as an upgrade, because being able to close a door in the first difficult week is a clinical need. The programme
is an adaptation of the methodology of PROMIS Clinics in the United Kingdom, which has treated addiction since
1986.

£9,900 for 28 days, published in full, with an itemised written quotation before you
commit. A medically supervised detox, where it is clinically needed, is completed in an appropriate medical
setting beforehand and billed separately, and that is true wherever you go in Spain. We pay and accept no
referral fees in either direction, so nobody earns a commission for sending you here.

## What Sierra Recovery is, and what it is not

Sierra is a registered health centre, N.I.C.A. 67331, care unit U.71, authorised by the Junta de Andalucía for supervised residential addiction treatment.

It is not a hospital and not a detox unit, which is true of most residential centres in
Spain and worth confirming wherever you look. Where a medically supervised withdrawal is clinically
needed it is completed in an appropriate medical setting before admission and billed separately. We do
not admit anyone who needs hospital-level monitoring or nursing, anyone acutely suicidal or in psychiatric
crisis, or anyone whose primary condition is a severe eating disorder. If that is the situation, we will say so
on the first call and point you somewhere that can help.

## Questions worth asking any luxury centre

- What is your registration number, and which register? Then check it.

- Can I see the written week? Not the therapy list. The timetable. How many therapeutic
hours, delivered by whom, with what qualifications.

- How many other clients will be there? And if the answer is none, ask how the group work
happens.

- Do the same clinicians treat me throughout? Rotation mid-stay costs more than people
realise.

- Does anyone receive a commission for referring me? Ask in writing. It is a federal crime
in the United States and merely legal in Spain, which is exactly why you should ask.

- What is included, itemised, in writing, before admission? And what is billed separately.

- What happens when I go home? Who delivers aftercare, for how long, and at what cost.

Not reliably, and the price is not the signal people think it is. Beyond a certain point the money is buying privacy, staff-to-client ratio and the building, not more or better therapy. The things worth asking about are the clinical programme, who delivers it, whether the same people treat you throughout, and what happens after you leave. Those questions all have answers you can ask for in writing, at any price point.

Are luxury rehabs in Spain licensed?

Some are and some are not, so it is worth checking the public register rather than the website. In Spain a residential centre treating addiction is a health centre and must be authorised by the health department of its autonomous community and entered on a public register. Ask any centre for its registration number and search it yourself in the regional register or in REGCESS, the national one. A company number, a VAT number or an association logo is not the same thing.

What is a therapeutic community, and why does it matter?

It is the group of people in treatment with you, and the structure around them. It matters because a large part of what changes in residential treatment happens between sessions: being known, being challenged by people in the same position, and practising ordinary life without the substance. A programme with one client cannot provide it, however good the setting is.

Do I need a private villa to recover?

No. You need a private room, which is a clinical need in the first difficult week, and all rooms here are individual. Beyond that, isolation is usually the opposite of what helps. Most people arrive having already tried to stop alone.
