Ibiza has one established residential rehab, a free public addiction service, and two English-language AA meetings a week. Beyond that, provision on the island is outpatient or off-island. The Balearics run their own health register under Decreto 100/2010, so any centre treating addiction here must be authorised by the Conselleria de Salut, and an Andalusian or mainland registration means nothing on this island. For many people the more useful question is not which centre on Ibiza, but whether to treat on Ibiza at all.

## The picture in Ibiza

Ibiza's economy runs on people drinking and taking drugs. That is not a moral observation, it is the industry, and it shapes everything about getting help here.

If you work in it, your colleagues use, your shifts end at six in the morning, your social life happens in the places you are trying to avoid, and everyone you know is having a season. The supply is not a phone call away, it is in the room. Meanwhile the island's population multiplies in summer and collapses in winter, and so does the demand on its services.

There is also a seasonal pattern we see every year. Somebody holds it together until October, the season ends, the structure disappears overnight, and the drinking that was functional in June is not functional in November. If that is where you are, the winter is the window.

## Free addiction services in Ibiza

Start here, before you consider paying for anything. We publish these because for a lot of people they are the right first step.

The Balearic health service, [IB-Salut](https://www.ibsalut.es/), provides addiction treatment through the public network, and the Consell d'Eivissa runs local drug dependency and prevention services. Treatment is free at the point of use if you are registered with the Balearic health service, and the route in is normally your centre de salut or a direct referral. Services are delivered in Spanish and Catalan, which is the practical limitation for most people reading this page in English.

If you are a seasonal worker who never registered, or a visitor, the public network is not designed for you, and a GHIC or EHIC covers state-provided necessary care rather than planned treatment. That is the honest reason many people in that position end up going home for treatment or paying privately.

## AA and NA meetings in Ibiza

This is the most immediately available thing on this page and it costs nothing.

An English-language Alcoholics Anonymous group meets on the island. Ibiza International meets at the Rosario Church on Avenida Isidor Macabich in Ibiza Town, on Wednesday evenings at 19:00 and Friday at 18:30. Times move, so confirm with the group or through the [European meeting finder](https://alcoholics-anonymous.eu/meetings/) before travelling across the island.

Two meetings a week is a real resource and worth using. It is also, honestly, thin compared with the mainland or the UK. If mutual aid is going to be the backbone of your recovery, that thinness is worth weighing when you decide where to do this.

### What actually happens at a first meeting

You turn up, you sit down, and you do not have to speak. Nobody takes your surname, nobody asks for money, and nobody will make you commit to anything. People share their own experience for an hour and then it ends. On a small island the anonymity matters more than usual, and it is taken seriously.

## Private rehab on Ibiza, and what it costs

There is one long-established residential centre on the island, a converted farmhouse estate treating substance dependency, behavioural addictions and mood disorders. It is a serious operation, and for some people it is the right answer, particularly if you have children here or a job you cannot leave.

Beyond it, what is available privately is outpatient: psychologists and psychiatrists in private practice, some working in English, German and other languages because the resident and visitor population demands it.

Ibiza also sits inside the wider Balearic luxury market, where single-client villa programmes are priced for an international clientele. Across Mallorca, Ibiza and around Marbella, that ultra-private end runs from roughly €47,380 to as much as €100,000 a week, while more conventional villa programmes on the Spanish coast start at about €4,500 a week, around €18,000 a month. ## Checking a centre is authorised in the Balearics

This is the check worth making, and the rules here are not the same as on the mainland.

Addiction treatment is a regulated health activity across Spain under Real Decreto 1277/2003. Each autonomous community then runs its own authorisation and register. In the Balearics that is [Decreto 100/2010](https://www.caib.es/seucaib/es/tramites/tramite/1540015), later modified by Ley 10/2018, which governs health authorisation and the Registre de Centres, Serveis i Establiments Sanitaris de les Illes Balears, administered by the Conselleria de Salut.

The practical consequence: a centre with beds on Ibiza must be authorised by the Balearic authority. An Andalusian N.I.C.A. number, a mainland registration, a company tax number or a certificate from an international membership association is not the same thing and is sometimes offered in its place.

### The documents that are not a health licence

When you ask, you may be shown a different document. On an island of holiday villas the distinction below matters more than anywhere.

- A tourist letting licence is not a health licence. On the mainland these are VUT numbers, vivienda con fines turísticos; the Balearics run their own holiday-rental permits. All of them authorise letting a property to visitors. None of them says anything about anyone's fitness to treat addiction. A villa operating on one is a legally let holiday house that happens to have therapists visiting.

- A licence in somebody else's name. Check whose entry it is. A registration held by a visiting psychologist, a separate outpatient clinic or an associated company does not cover the house you would be living in. It has to belong to the centre at the address where you would stay.

- A psychology practice licence. An outpatient psychology consultation is a real health registration and is not authorisation to run a residential addiction programme. What you need is an entry authorising addiction care specifically.

- Private accreditations and memberships. Certificates from international associations can be worth having, and they are separate from the legal register.

What to ask, in writing:

- The registration number and the exact registered company name, so you can look it up in the register yourself. Trading names and registered names are often different, which is the point of asking for the registered one.

- Which register it belongs to, and whether that register covers the island the beds are actually on.

- What the entry authorises, and when it expires. Authorisation for an outpatient service is not authorisation for a residential unit, and neither of them authorises a medical detox.

- Who provides medical cover, physically where they are, and at what hours. Then ask what happens at three in the morning if somebody has a seizure.

One thing to be clear about: the assistance-unit codes differ by region, so do not go asking an Ibiza centre for an Andalusian code. What transfers is the principle. Every region issues an authorisation naming the clinical units a centre may operate, addiction care is one of them, and the entry either exists in that region's register or it does not. A centre that holds an authorisation answers the first question immediately, because it is public information. Evasion is itself the answer, and finding that out costs you nothing.

## Why treating off the island is often the point

Residential treatment works partly by removing you from the cues attached to your using: the route home, the colleague, the hour of the evening when it always starts. On an island the size of Ibiza there is a limit to how far you can be removed.

You can be in a beautiful finca in the hills and still be twenty minutes from every bar you have ever drunk in, still reachable by everyone who expects the old version of you, and still inside the same season. Distance does real clinical work, and Ibiza does not have much distance available.

That is not an argument that the island's provision is poor. It is an argument about geography, and it is the reason a great deal of Ibiza's demand has always travelled to the mainland or back to the UK.

## Sierra Recovery, Málaga, Spain

We are not on Ibiza. The centre is on the mainland in Málaga province, at Cortijo Ferradores in Humilladero near Antequera, about forty five minutes from Málaga airport. Ibiza connects to Málaga directly in season and via Palma, Barcelona or Madrid the rest of the year, and we arrange the transfer at the Málaga end.

Because we are in Andalusia, we are on the Andalusian register, under N.I.C.A. 67331, authorised for atención sanitaria a drogodependientes (healthcare for drug dependency, authorised care unit U.71): a health centre with a supervised treatment residence. That is expressly not a hospital and not a detox unit, which is why detox, where clinically needed, happens in a hospital before admission and is billed separately. The programme is twenty eight days, with three structured group sessions a day plus an afternoon group activity, three one-to-one sessions a week, and a seventeen-module curriculum. The method is adapted from [PROMIS Clinics](/about/), who have treated addiction in the United Kingdom since 1986. The therapy is evidence-based rather than proprietary: cognitive behavioural therapy at the core, with dialectical behaviour therapy, EMDR and somatic work where trauma sits underneath the using, family therapy, and relapse prevention agreed before discharge. Co-occurring depression, anxiety, PTSD and burnout are treated in the same plan. We do not treat eating disorders as a primary condition.

The published price is £9,900 for twenty eight days with a private room included, with hospital detox and medical costs billed separately and an itemised quotation before you commit. We neither pay nor accept referral fees.

One practical warning if you are coming from a season: the thing to prepare for is not the therapy, it is the silence. There is no music here and nothing happens at four in the morning. For the first week that is genuinely difficult for people who have organised years around the opposite. It gets easier, and it is doing something.

## Which option is right for you

Start with the free services if you are registered in the Balearics and can manage the process in Spanish. For many people the public network plus meetings is enough on its own.

Use the meetings regardless. Two a week in English is what is there, it is free, and it is what will still be there in month six.

Consider the island's residential centre if you cannot leave Ibiza because of children or work, and apply the authorisation checks above.

Consider leaving if the island itself is part of the problem, if the season is what you are trying to get out of, or if you have tried locally and it has not held.

If you are physically dependent, the order changes: medical advice first, detox in a hospital, then a programme. Do not plan to dry out on the ferry.
