# Rehab in the UK vs Spain: an honest comparison

A straight look at cost, distance, climate and clinical care, written to help you choose well, including the times a UK service is the better answer for you.

For many UK adults, a residential programme in Spain costs roughly a third of an equivalent UK private stay. ## What you actually pay

Cost is usually the first real difference people notice, and it is a large one. UK private residential rehab runs at roughly £12,000–£40,000 per month on public listings, and there is very little credible under £10,000 a month. It helps to understand why the number is lower, because "cheaper" can sound like "lesser" and that is not what is happening here. Four things drive the difference: the Spanish cost of living sits well below the UK; the house is in rustic countryside, not central London; there is no luxury mark-up; and we pay no referral fees. On that last point, brokers and some "impartial" helplines are quietly paid a commission to steer enquiries, sometimes a substantial share of the fee. One honest caveat: with a Spanish programme you also budget for flights, and if a medically supervised detox is needed first, that is arranged and billed separately, the same as it would be under most UK residential models. ## Distance from home: a barrier, or the point?

The instinct is that treatment close to home is easier. Sometimes it is. But a residential stay usually means leaving home anyway, and for many people the distance from the exact streets, contacts and routines tied to their using is part of what makes early recovery possible rather than a drawback. A short flight to Málaga puts real space between you and the environment where the habit was learned, without putting you on the other side of the world.

The climate is a genuine, if secondary, factor. Long daylight, warmth and being able to walk in the hills most of the year support sleep, routine and mood at a time when all three are fragile. It is not a holiday, and we do not sell it as one, but the setting does real work. Distance cuts the other way too, and we say so below. If you have young children at home, a job you cannot step away from, or clinical needs that call for a hospital nearby, closeness matters more than climate.

## The clinical model travels, wherever you go

The thing that should decide your choice is the quality of the clinical work, not the postcode. Sierra's programme is built on the [Regulation Model](/the-regulation-model/), the approach used by PROMIS Clinics in the UK, who have treated addiction since 1986. The model treats addiction as a learned way of regulating an unbearable internal state, not as a moral failure or a lack of willpower. That framing changes the whole tone of the work: it is skills-based and non-shaming rather than confrontational.

In practice that means structured, skills-based psychoeducation running through the 28 days: identifying and challenging the beliefs that drive a reaction, building emotional regulation and distress tolerance, setting boundaries, and rehearsing relapse-prevention skills you can use under pressure. On regulation and safety, be precise when you compare centres, in the UK or in Spain. Sierra is a registered health centre with the Junta de Andalucía (NICA 67331). It is not a hospital and not a detox unit, which is why any detox is completed elsewhere first, with qualified nurses part of the team during your stay. ## When a UK option is the better choice

Spain is not the right answer for everyone, and choosing it when a UK route fits you better helps no one. A UK service is likely the better call when:

- You need medically complex inpatient detox with a hospital on the same site, rather than detox completed before a residential programme.

- Your first and free step has not been tried yet. NHS and local-authority community services, and groups like AA and NA, cost nothing and are the right starting point for many people. You do not have to pay to begin.

- Being physically near children, a carer role or work commitments matters more to you than distance from your triggers.

- Flying is not realistic for health or practical reasons.
