What rehab in Spain actually costs at our treatment centre

Here is the starting price, up front: exactly what it includes, and what changes it, so you can make a decision without a sales call first.

Residential rehab at Sierra Recovery costs £9,900 for the 28-day programme, published in full rather than quoted on request. That figure includes a private room as standard, all meals cooked by a Michelin-trained chef, three structured therapy groups a day five days a week, individual therapy three times a week, the seventeen-module psychoeducation curriculum, holistic and equine work, and a full year of aftercare: one session a week, online or in person at the PROMIS clinics in London, which is more than 48 sessions in the twelve months after you leave. It does not include a medically supervised detox, which is completed in an appropriate medical setting before admission and billed separately, nor flights. You receive an itemised written quotation before you commit, and no referral fee is paid or accepted in either direction. For comparison, published directory prices put a comparable private stay at roughly £12,000 to £40,000 in the UK, $20,000 to $40,000 in the United States, and A$30,000 to A$50,000 in Australia.

Why it costs less

The same standard of care, at a fraction of the price. Here is why.

Why is it a fraction of the price in the UK, the US or Australia? Because almost everything it costs to run a clinic is cheaper in rural Spain, and none of it is the care itself.

  • The cost of living is far lower. Staff, food and the day-to-day running of the centre cost a fraction of what they do in a UK city, and rural Andalucía is cheaper again than Spain’s cities. That difference flows straight into the price you pay.
  • Countryside, not central London. We are a restored farmhouse on rural land, not a clinic carrying prime city-centre rent and rates.
  • No luxury mark-up. We are not a luxury brand, so you are not paying for one, and there is no expensive advertising machine baked into your fee.
  • No referral fees, ever. Across the industry, brokers and “impartial” helplines are quietly paid a commission, sometimes as much as 30% of your fee, to steer you to a particular clinic. In the United States that is a federal crime; in the United Kingdom it is not yet illegal, but it is widely regarded as unethical patient brokering, and the advertising regulator has already ruled against helplines that concealed it. You come to Sierra directly, so not a penny of your money goes to a middleman, and all of it goes into your treatment. Our founder, Robin Lefever, has long been one of the voices against referral fees in UK addiction treatment.

What does not change is the clinical standard. The programme is an adaptation of the PROMIS Clinics methodology, built for this centre and clinically backed by PROMIS in the United Kingdom. To be clear about what that does and does not mean: this is not the PROMIS programme sold at a lower price. It is a different centre, in a different country, with a different setting, working to a model adapted from theirs. Sierra gives you a licensed clinical programme and your own private room for £9,900, roughly a third of a comparable residential stay in the UK, and less again against the US, in a warm climate, with food from local produce and real distance from the places and people tied to using. What you are not paying for is a luxury setting, because this is not one. Affordable does not have to mean lesser.

Comparing the market

What the luxury rehabs and villas actually charge for in Spain

You will see villas advertised along the Costa del Sol, in Marbella and on the islands at £30,000 to £50,000 a month, and in some cases well over £100,000. Much of that price is not clinical. A luxury villa rehab is usually a rented high-end house taking only three to five guests at a time, so a very high rent and a large staff are spread across a handful of people, and the nightly rate climbs to match. A good part of what you pay is the villa and the exclusivity, not a better clinical outcome.

Two questions are worth asking any Spanish centre before you pay:

  • Is it a registered health centre? In Spain, providing health care for a drug dependence requires an authorisation from the regional health authority, entered in a public register anyone can search. Some high-end villas hold only a tourist-accommodation licence and bring the therapy in through outside professionals. An accommodation licence is not a health authorisation.
  • Who holds the clinical responsibility, and can they show it? Ask to see the health authorisation and the registration number, then check it yourself.

Common questions about the cost

Why do you publish the price when others do not?

Because being made to ring up and negotiate is the last thing anyone needs at this point. Publishing the starting price is more honest, and it means you can make a decision without a sales call first.

Is a lower price a lower standard of care?

No. The difference is the cost of running a clinic in rural Spain rather than in the UK, and the fact that Sierra is not a luxury centre and does not charge for one. The programme is delivered by a qualified, registered clinical team working to a model adapted from PROMIS Clinics, who have treated addiction in the UK since 1986. High clinical standards and an unfussy setting are not the same thing, and it is the setting we have kept simple, not the clinical care.

Is this the same programme as PROMIS in the UK?

No, and we would rather say so plainly than let the price invite that conclusion. Sierra runs its own programme, adapted from the PROMIS Clinics methodology and clinically backed by PROMIS, in a different country and a different setting. If you want the PROMIS programme itself, that is delivered at their own clinics in London and Kent, and it is priced accordingly. What you get here is the adapted model, delivered by our own registered clinical team under our Andalusian health authority licence.

Do you take insurance?

Sierra Recovery is primarily a private self-pay centre, but we work with international private medical insurance. Some policies may contribute toward eligible clinical services, subject to the insurer’s prior written authorisation; residential accommodation and non-clinical services may not be covered. Tell us who your insurer is when you get in touch and we will tell you honestly what is likely to apply.
